Designation of Beneficiary

Every new and returning member must complete a Member Sworn Statement, which is an affidavit on which a member provides important demographic and employment information to VCERA. On the statement, a member also will designate one or more beneficiaries to receive a benefit when he or she dies. Because of the community property laws in California, married members typically name their spouse as their primary beneficiary to receive survivor benefits.

Please keep your beneficiary designations current. Notify VCERA if you marry or divorce, if you register a domestic partnership, or if your beneficiary dies. Completing a new Beneficiary Designation Form will supersede all prior designations. Click here to download VCERA’s beneficiary form.
